I REQUIRE A MINIMUM OF 48 HOURS NOTICE TO CANCEL OR RESCHEDULE AN APPOINTMENT. FAILURE TO NOTIFY WILL RESULT IN A 50% CHARGE OF SERVICE
• MISSED APPOINTMENTS
FAILURE TO CANCEL AN APPOINTMENT AND/OR NO-SHOW WILL BE REQUIRED TO PAY 75% OF THE SERVICE SCHEDULED
